From Monday 20th January, the referral details for some community services in Blackburn with Darwen will change. These services are,
- Community rehabilitation (including occupational therapy and community physiotherapy)
- District nurses
- Integrated neighbourhood teams (and other associated services)
- Pulmonary rehabilitation
- Community stroke team
From that date, you will need to send any new referrals directly to the following: –

Please note, there are no changes to the SPA referral form other than to include the new referral details for each service.
