Spring North on behalf of The VCFSE Alliance is pleased to announce three funding opportunities under the UEC Winter Capacity programme. These opportunities aim to enhance support for vulnerable populations across Blackpool and Blackburn with Darwen, particularly during the critical winter months. Each opportunity focuses on targeted interventions designed to address specific health and social care challenges in our communities with the primary aim of keep people out of hospital during the winter period by providing targeted interventions and support.

Funding Opportunities:

Targeted Support and Engagement for Adults with a Respiratory Condition: The projects primary aim is to keep people out of hospital during the winter period by providing targeted interventions and support to adults in Blackpool suffering from respiratory conditions, particularly COPD. The initiative will involve a comprehensive outreach and engagement strategy, alignment with existing respiratory and health services, and the development of community-based support networks.

The project will focus on addressing both clinical and non-clinical factors affecting respiratory health, such as food poverty and housing conditions, and will promote preventive health messaging around vaccinations, as well as collaborating with other local partners and organisations to avoid duplication of effort and offer added value, support people into existing services where there are some, and addressing unmet need if there aren’t existing services.


Targeted Support for Older People in Darwen Who Are Frail and at Risk of Falls: The projects primary aims are to keep older people out of hospital and reduce the frequent use of urgent and emergency care (UEC) services during the winter period by providing targeted interventions and support to older people in Darwen at risk of injury due to falls. The initiative will focus on supporting frail older adults, particularly those at high risk of falling, whether they reside at home, in a care home setting, or are active in the community.

The project will involve proactive outreach, the development of preventive interventions, and close collaboration with local health and social care partners and organisations to avoid duplication of effort and offer added value, support people into existing services where there are some, and addressing unmet need if there aren’t existing services.


Targeted Support for Individuals Facing Multiple Disadvantage in Blackburn Central Ward: The projects primary aims are to keep people out of hospital and reduce the frequent use of urgent and emergency care (UEC) services during the winter period by providing targeted interventions and support to individuals aged 20-64 who face multiple disadvantages in Blackburn Central Ward. The initiative focuses on identifying and supporting individuals and families who have high UEC attendance but do not meet the threshold for referral to the Changing Futures programme.By working collaborative with other local partners and organisations to avoid duplication of effort and offer added value, support people into existing services where there are some, and addressing unmet need if there aren’t existing services, the project seeks to understand and address the underlying causes driving demand for UEC services, improve access to broader health and social care services, and reduce unnecessary UEC attendances.

Sorry for the short notice – but the funding opportunity has only just been issued and the deadline is 10am on Monday 2nd September.

Community CVS would like to invite VCFSE Sector organisations already working in Blackburn with Darwen who would like to be part of a CVS led bid for Lot C to a workshop.

Your organisation will need to have a track record of engaging with and/or working with adults aged 20 to 64 year old experiencing multiple disadvantage (e.g. homelessness, experience of the criminal justice system, mental health, alcohol or substance misuse, survivor or perpetrator of domestic abuse) who reside within the Blackburn Central ward.

You will need to bring with you to the workshop information on how your organisation can work collaboratively to

i) Identify and engage the target cohorts of individuals and families living in Blackburn Central who are at risk of frequent use of urgent and emergency care;

ii) Help to identify the underlying issues driving by high urgent emergency care demand within Blackburn Central and involve people with lived experiences to understand the factors contributing to this demand;

iii) Suggest interventions that will address both immediate needs and underlying causes;

iv) Suggest ways of monitoring and evaluating the effectiveness of the interventions to reduce urgent and emergency care attendances;

v) Suggest ways of gathering service user feedback and using the feedback to refine interventions and improve service delivery”

The workshop is being held on Tuesday 27th August from 10am to 12 noon within the Conference Room at the Boulevard Centre, 45 Railway Road, Blackburn BB1 1EZ.

Delivery Period: The funding opportunities cover a 7-month period, including a 1-month mobilisation phase and 6 months of direct service delivery, from September 2024 to 31st March 2025.
